It's saying I have promoted the same player twice to my seniors and I have lost a promotion chance because of it. I have 1 left instead of 2

I have 6 available slots to promote with my academy every season. 4 of these players I have promoted already. 1 of these players has taken 2 promotion slots from my academy not 1 like it should be.
