I think I understand lopsided tactics, but I want to make sure. If, for example, a team runs several consecutive first down SG-4 rushes, the defense adapts by shifting to the rush defense preference, right? Is that all there is to it, or do defensive players also perform better because they are anticipating the run?

Also, can defenses run lopsided tactics? Meaning, if every play, the opponent uses a 3-4-4 with 75% LB blitz, will the offense get any better at playing against that defense during the game? Thanks.

There should be a dot instead of a comma between part & Defense as otherwise there is a possibility to misread it:

This calculation happens only on the Offensive tactics part, Defense and Special teams are not subject of this calculation.

There are two mechanism in here.

The Defense AI lets switch the EXPECTATION from pass to run in your example. The Lopsided tactics penalty won't do anything on this one. But if the majority of the plays would result in a SG4WR, the lopsided tatcics penalty would weaken the whole offense...

correct me if i am wrong, but this is connected to coaches karma isn't it?

I have noticed that i get thumbs down even when i play all 9 or 10 offensive formations during the game, but the opposing defence plays 1,2 or 3 defensive formations.

Then you will see the opposing offence play less offense formations and i play 8 different defensive formations.

I'm not quite sure what I can do to get thumbs up.
