A nuclear weapon gives off a "signature" if you will. It has to do with various impurities and such in the fissionable or fusion materials. So I have no doubt that the various intelligence agencies around the world got a good view of the N. Korean nukes, including the one that sort of "squibbed" when it didn't fire properly. Even underground tests give a signature from what I can gather.

N. Korea has made no secret of its desire to nuke someone, apparently the USA is at the top of that list. Maybe their "dear leader" thinks a nuke will cause the entire country to surrender to him.

What seems more likely is that several "boomer" submarines, amongst other nuclear capable weaponry, are ready, willing and able to deliver a fast response.

To be sure the first round would go to the North Koreans. They'd get a base, maybe two that belong to the US. From what I can get the North Koreans have missiles capable of reaching US Pacific bases. Whether or not they have a nuclear weapon small enough to mount on one of those missiles is open for some debate. Of course missile delivery is only the fastest method.

Who's to say they couldn't crate the thing up, transship it somewhere, and have it arrive in a cargo container? However this would not give the North Koreans any immunity from attack, since the various missile subs (boomers) are difficult if not impossible to find when they are on patrol.

But could this happen? Well let's assume, for the sake of argument, that they have a weapon small enough for one of their intermediate range missiles. Now in the USA, and other major nuclear powers, there are all kinds of Permissive Action Links that must be gone through in order to release and launch nukes. This is a good thing, helps to prevent a "Dr. Strangelove" scenario.

But do the North Koreans have such controls? Not sure about that. And of course there is the "Bedford Incident" scenario to consider if the controls are lacking. (The "Bedford Incident" was a very scary movie in which a US Navy destroyer is tracking a Russian sub. The destroyer captain has his entire crew wound up to a fever pitch, and when talking to a reporter on the ship (played by Sidney Poitier) the captain says, "If he fires one, I'll fire one." At which point one of the crew, wound up, fires a nuke depth charge (ASROC) at the Russian, who fires nuclear torpedoes back.)

So if you have a North Korean military man who hears his "Dear Leader" say he wants to launch, and then does so, what follows is an attack. The US has the capability to track missile launches from countries, which is why space launches from the major nuke powers are always announced in advance. So the launch would be detected. They might even decide to ride it out, to see if the thing goes off. But the response towards North Korea would be, to use a phrase, assymetrical to say the least.

Nothing good can come of this.

The best simile I can come up with is a four year old sitting there with a loaded gun in his hands, and the hammer is cocked. Someone is going to get hurt.
